Add review_duration_ms:int and output_modified:bool to PipelineRun. An instant rubber-stamp is meaningfully different from a 90s review-with-edits for ND users. Critical for detecting when a crystallised workflow is being approved too fast to be genuinely reviewed. From cf-orch#17 Minor gap #8.
