Redesign annotation stream to show per-speaker conversation threads.

Requirements:

  • Left/right or color-coded thread layout (like a chat UI)
  • Speaker color/avatar assignment is local only — no names stored, no biometric data
  • User can rename speakers locally for the session (e.g. 'Me', 'Doctor')
  • Tone annotation appears inline in each speaker's thread
  • Accessible: speaker distinction must not rely on color alone (add shape/label)
